ABSTRACT

Objetivo: Describir las características clínicas y patológicas del carcinoma basocelular. Métodos: Se realizó un estudio descriptivo y retrospectivo de serie de casos en el Servicio de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer", desde enero del año 2016 a diciembre de 2018. Se incluyeron todos los pacientes con diagnóstico histopatológico posoperatorio de carcinoma basocelular en el período de estudio. La muestra quedó conformada por 54 pacientes y se caracterizó según el año del diagnóstico, la edad, el sexo, el color de la piel, la procedencia, el grado de diferenciación celular, el párpado afectado, la localización palpebral, el diagnóstico preoperatorio y el diagnóstico histopatológico posquirúrgico. Resultados: El 57,4 por ciento de los pacientes fueron del sexo femenino. El grupo más afectado fue el de 60-79 años con el 53,7 por ciento de los casos, con una edad media de presentación de 54,57 años. El 88,9 por ciento de la muestra fueron de piel blanca. En el 79,6 por ciento se afectó el párpado inferior y en el 64,8 por ciento el tercio interno. El 64,8 por ciento fueron carcinoma basocelular poco diferenciados y la concordancia entre el diagnóstico clínico y el histopatológico fue del 85,2 por ciento. Conclusiones: El carcinoma basocelular es más frecuente en el sexo femenino, en los adultos mayores y en los pacientes de piel blanca. En cuanto al grado de diferenciación celular, es poco diferenciado, y se localiza fundamentalmente en el tercio interno del párpado inferior. Existe concordancia entre el diagnóstico clínico y el histopatológico en la mayoría de los pacientes(AU)


Objective: Describe the clinical and pathological characteristics of basal cell carcinoma. Methods: A retrospective descriptive study was conducted of a case series from the Oculoplastic Surgery Service at Ramón Pando Ferrer Cuban Institute of Ophthalmology from January 2016 to December 2018. The study universe was all the patients with a postoperative histopathological diagnosis of basal cell carcinoma in the period analyzed, from whom a sample 54 patients was selected. The variables analyzed were year of diagnosis, age, sex, skin color, place of residence, degree of cell differentiation, eyelid affected, eyelid location, preoperative diagnosis and postsurgical histopathological diagnosis. Results: Of the patients studied, 57.4 percent were female. The most affected age group was 60-79 years with 53.7 percent of the cases and a mean presentation age of 54.57 years. 88.9 percent of the sample had white skin. The lower eyelid was affected in 79.6 percent and the inner third in 64.8 percent. 64.8 percent were poorly differentiated basal cell carcinomas, whereas agreement between clinical and histopathological diagnosis was 85.2 percent. Conclusions: Basal cell carcinoma is more common in the female sex, elderly people and white skin patients. In terms of degree of cell differentiation, it is poorly differentiated, and its main location is the inner third of the lower eyelid. In most patients there is agreement between the clinical and histopathological diagnosis(AU)

RESUMEN Objetivo: Evaluar la concordancia entre el diagnóstico clínico inicial y el histopatológico de tumores palpebrales malignos. Métodos: Se realizó un estudio observacional, descriptivo y retrospectivo de pacientes portadores de diferentes tumores palpebrales, atendidos en el Servicio de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer", en el periodo comprendido de enero a diciembre del año 2016, en el que se describieron las características clínicas y anatomopatológicas, y la concordancia entre ambos diagnósticos. Resultados: Predominó el grupo de 60-79 años de edad (46,1 por ciento), el sexo femenino (51,7 por ciento) y el color de piel blanco (92,6 por ciento) . Las lesiones se presentaron fundamentalmente en el párpado inferior (77,0 por ciento) y las telangectasias constituyeron el signo de malignidad más consistente (56,8 por ciento), con predominio del carcinoma basocelular, seguido del carcinoma espinocelular (81,4 por ciento y 15,2 por ciento, respectivamente). La concordancia entre el diagnóstico clínico y el histopatológico fue de 69,2 por ciento para el carcinoma basocelular; 9,7 por ciento para el espinocelular y 30 por ciento para el sebáceo (índice de Kappa de 0,338; 0,262 y 0,357, respectivamente). Conclusiones: Existe la mayor concordancia entre diagnóstico clínico e histopatológico en el carcinoma basocelular; no así en el resto de los tumores estudiados(AU)


ABSTRACT Objective: To assess the concordance between the initial and histopathological clinical diagnosis of malignant palpebral tumors. Methods: An observational, descriptive and retrospective study of patients with different eyelid tumors was performed. The patients were treated in the Ocular Plastic Surgery Service of Ramón Pando Ferrer Cuban Institute of Ophthalmology, in the period from January to December of 2016, during which the clinical and pathological characteristics were described, together with the concordance between both diagnoses. Results: The age group of 60-79 years predominated (46.1 percent), as it was with the female sex (51.7 percent) and the white skin color (92.6 percent). The lesions occurred mainly in the lower eyelid (77.0 percent) and telangiectasia constituted the most consistent sign of malignancy (56.8 percent), with a predominance of basal cell carcinoma, followed by spinocellular carcinoma (81.4 percent and 15, 2 percent, respectively). The concordance between the clinical and histopathological diagnosis was 69.2 percent for basal cell carcinoma; 9.7 percent for the spinocellular carcinoma; and 30 percent for the sebaceous carcinoma (Kappa index of 0.388, 0.262 and 0.357, respectively). Conclusions: There is the greatest concordance between the clinical and histopathological diagnosis in basal cell carcinoma, but not in the rest of the tumors studied(AU)

ABSTRACT

Introdução: O melhor método para reconstruir uma unidade anatômica é substituir a área lesada pelos mesmos tecidos. Métodos: Três pacientes que apresentavam carcinomas basocelulares na pálpebra inferior foram submetidos à excisão, seguida de reconstrução imediata com a transposição em espessura total de tecido da pálpebra superior em um só tempo. Resultados: Os pacientes evoluíram quase sem complicações, as pálpebras mantiveram suas funções de fechamento total e proteção do globo ocular. Conclusão: O retalho se mostrou factível e é mais uma alternativa para reconstruir a pálpebra inferior.


Introduction: The best method to reconstruct an anatomical unit is to replace the injured area with the same tissues. Methods: Three patients with basal cell carcinoma in the lower eyelid underwent full-thickness lower eyelid excision, followed by immediate reconstruction with a single-stage fullthickness transposition of the upper eyelid tissue. Results: The patients had almost no complications, and the functions of total closure and protection of the eyeballs of the eyelids were maintained. Conclusion: The flap proved to be feasible and is an alternative choice for reconstruction of the lower eyelid.

ABSTRACT

Los principales tumores malignos que afectan los párpados en orden de frecuencia son el carcinoma basocelular, el carcinoma de glándulas sebáceas, el carcinoma de células escamosas y el melanoma maligno. El carcinoma de glándulas sebáceas es una neoplasia originada en las glándulas sebáceas con predilección por cabeza y cuello, especialmente por las glándulas de Meibomio, aunque también puede afectar a las glándulas de Zeiss o ambas y su sitio de mayor presentación es en el párpado superior. Se presenta un paciente masculino de 46 años de edad quien presenta lesión pediculada y vascularizada en párpado inferior izquierdo posterior al drenaje de un supuesto chalazión. Se realiza rasurado de la lesión y recidiva por segunda ocasión. El carcinoma de células sebáceas se maneja con resección amplia y reconstrucción con técnica de Hughes(AU)


The main malignant tumors affecting the eyelids in an order of frequency are basal cell carcinoma, sebaceous gland carcinoma, squamous cell carcinoma and malignant melanoma. Sebaceous gland carcinoma is a neoplasm originating in sebaceous glands predominantly from the head and neck, particularly meibomian glands, though it may also affect the glands of Zeis or both, and its most common site of presentation is the upper eyelid. A 46-year-old male patient presents with a pediculated vascularized lesion on his lower left eyelid after drainage of a supposed chalazion. The lesion was shaved off and reoccurred a second time. Sebaceous cell carcinoma is managed with broad resection and reconstruction by Hughes' technique(AU)

ABSTRACT

RESUMO O objetivo dos autores é relatar um caso de fibroma de tarso, uma tumoração na reborda orbitária súpero-temporal direita com crescimento lento ao longo de anos. A análise da peça obtida da região tarsal mostrou uma lesão bem delimitada, constituída por deposição de fibras colágenas densas, com proliferação de células fusiformes, sem atipias, com revestimento de células conjuntivais, caracterizando fibroma da placa tarsal.


ABSTRACT The objective of the authors is to report the case of a tarsal fibroma, a tumoration on the superotemporal side of the right orbit with slow growth over the years. Analysis of the anatomical specimen obtained from the tarsal region showed a well-defined lesion, consisting of deposition of dense collagen fibers, with fusiform cells without atypia, with coating conjunctival cells, featuring fibroma of the tarsal plate.

ABSTRACT

Objetivo: Analisar a acurácia do exame clínico no diagnóstico de lesões palpebrais. Métodos: A partir da observação de trinta e cinco fotos de tumores palpebrais benignos e malignos que foram apresentadas a médicos oftalmologistas, para cada foto foram feitas 3 perguntas: 1) lesão maligna ou não? 2) se considerado maligno, o provável tipo histológico; e 3) tumor agressivo ou não? Os médicos foram agrupados em 9 grupos, de acordo com a formação profissional (tempo de formação e especialização ou não em Oculoplástica). As respostas foram comparadas com o resultado do exame histopatológico da peça retirada cirurgicamente. Resultados: No total, cento e seis médicos oftalmologistas participaram do estudo. A experiência do profissional influenciou no diagnóstico de malignidade do tumor, já que o grupo 1 (residentes de primeiro ano) apresentou a menor acurácia (64,5%), com menor concordância estimada (Kappa = 0,13), e o grupo 5 (formados há 5 anos e especializados em Oculoplástica) a maior acurácia (77,3%), com melhor concordância (Kappa = 0,45), além de apresentar as melhores medidas para os demais itens avaliados. Para diagnóstico do tipo histológico, a acurácia foi menor no grupo 1 que obteve o pior desempenho, com 51,1% de acurácia, enquanto o melhor foi o grupo 6 (formados há mais de 5 anos e especializados em Oculoplástica, 77,2%). Já para o critério de agressividade do tumor os resultados foram mais próximos entre as diferentes categorias. Os oftalmologistas formados há mais tempo e sem especialização em Oculoplástica também demonstraram baixa acurácia diagnóstica no diagnóstico de malignidade e na determinação do tipo histológico do tumor. Conclusão: A baixa acurácia no diagnóstico clínico de tumores palpebrais nos grupos acima referidos reforça a necessidade de melhorar o conhecimento em oncologia palpebral nesses grupos. .


Objective: To analyze the accuracy of the clinical examination in the diagnosis of eyelid lesions. Methods: From the observation of thirtyfive photos of benign and malignant eyelid tumors were presented to ophthalmologists, for each picture, it was asked 3 questions: 1) the lesion is malignant or not; 2) if considered malignant, the probably histological type; and 3) the tumor is aggressive or not. The phisicians were divided into 9 groups, according to academic degree (time since graduation and Oculoplastics specialty or not). Answers were compared with the results of the histopathologic study of the surgically resected tumor. Results: In total, one hundred and six ophthalmologists were interviewed. The professional experience influenced the diagnosis of malignancy of the tumor, as the Group 1 (first year residents) had the lowest accuracy (64.5%), with lower estimated agreement (kappa = 0.13), and Group 5 (graduated 5 years ago and with expertise in Oculoplastics) the highest accuracy (77.3%), with better agreement (Kappa = 0.45), and presented the best parameters for other analyzed items. For the histological type diagnosis, accuracy was lower: group 1 had the worst performance, with 51.1% accuracy, while the best was for group 6 (graduated over 5 years and with expertise in Oculoplastics, 77.2% ). As for the criterion of tumor aggressiveness, the results were closer among the different categories. Ophthalmologists trained longer and without expertise in Oculoplastics also showed a low diagnostic accuracy for malignancy and to determine the histological type of tumor. Conclusions: The low accuracy of clinical diagnosis of eyelid tumors in the groups above reinforces the need to improve ophthalmic oncology knowledge in these groups. .

ABSTRACT

El lupus eritematoso es una dermatosis inflamatoria, crónica, aguda o subaguda que evoluciona por brotes. Puede afectar a todos los órganos pero más frecuentemente a la piel. Presentamos a dos pacientes con lesión en forma de placa, localizada en zona interna y media del borde libre palpebral inferior, de bordes elevados, depresión central con fondo costroso y pérdida de las pestañas a ese nivel. Se decide -por la localización, extensión de la lesión, los antecedentes personales y la aplicación de tratamientos previos sin mejoría del cuadro clínico- realizar biopsia incisional para un mejor estudio. Los resultados anatomopatológicos revelaron el diagnóstico de lupus discoide palpebral. La evolución posterior al tratamiento médico fue satisfactoria, sin recidiva de la lesión durante el primer año de seguimiento.


Erythematosus lupus is an inflammatory dermatosis of chronic, acute or subacute nature that occurs in outbreaks. It can affect all the body organs, but mainly the skin. Here are two patients with plaque-form lesions located in the internal middle area of the free inferior palpebral border, thick contours, central depression with scabby bottom and loss of eyelash. Taking into account the location, the extension of the lesion, personal histories and application of previous treatment without improved clinical picture, it was decided to perform incisional biopsy for better examination. The anatomopathological results showed the diagnosis of palpebral discoid lupus. The recovery of the patient was satisfactory after the medical treatment, with no relapse during the first follow-up year.

ABSTRACT

OBJETIVO: describir las alteraciones morfológicas y morfométricas mediante microscopia confocal en el tejido corneal expuesto al aceite de silicona en pacientes vitrectomizados antes y después de la cirugía de catarata. MÉTODOS: estudio prospectivo, descriptivo, longitudinal y observacional en 34 ojos de pacientes operados de catarata por facoemulsificación con el uso de la técnica de facochop por córnea clara, previamente vitrectomizados con aceite de silicona de 1 000 centistokes en cavidad vítrea que presentaban aceite emulsionado en cámara anterior, a los cuales se les realizó microscopia confocal previa y posteriormente a la cirugía de catarata con microscopio confocal CONFOSCAN S-4/NIDEK de cuarta generación en el período comprendido entre febrero y diciembre de 2012. RESULTADOS: predominó el sexo masculino (64,7 %). El mayor número de casos estuvo en el rango de edades entre 39 y 58 años (47 %). La densidad celular endotelial fue más baja a nivel superior que a nivel central de la córnea que coincide con el área de mayor exposición al aceite de silicona tanto posvitrectomía pars-plana (VPP) como poscirugía de catarata con pérdida celular promedio de 358 células/mm2 (14,7 %). Los valores de pleomorfismo y polimegatismo fueron patológicos después de la cirugía secuencial. Hubo lesiones endoteliales concomitantes en el 100 % de la muestra. CONCLUSIONES: la microscopia confocal es una herramienta novedosa de gran eficacia para la detección y seguimiento de las modificaciones iniciales en el endotelio corneal que permanece en contacto con el aceite de silicona emulsionado en los pacientes vitrectomizados antes de que aparezcan signos clínicos de queratopatía


OBJECTIVE: to describe with the confocal microscopy the morphological and morphometric changes occurred in the corneal tissues exposed to silicone oil in vitrectomized patients before and after cataract surgery. METHODS: a prospective, descriptive, longitudinal and observational study was carried out in 34 eyes of patients operated on for cataract by phacoemulsification using the phacochop technique for clear cornea. These patients had been previously vitrectomized in anterior chamber with 1 000 centistoke emulsified silicone oil and had been performed confocal microscopy with 4th generation confocal microscope CONFOSCAN-S-4 NIDEK before and after cataract surgery in the period of February to December 2012. RESULTS: Males prevailed (64,7 %). The highest number of cases aged 39 to 58 years (47 %). The endothelial cell density was lower at upper level than at central level of the cornea, which agreed with the most exposed area to silicone oil after pars-plana vitrectomy and after cataract surgery. The average cell loss was 358 cell/mm2 (14,7 %). Pleomorphism and polymegatism values were pathological after the sequential surgery. There were concomitant endothelial injuries observed in 100% of cases. CONCLUSIONS: The confocal microscopy is a greatly effective innovative tool for the detection and follow-up of initial changes in the corneal endothelium that remains in contact with the emulsified silicone oil in vitrectomized patients before the onset of clinical signs of keratopathy.

ABSTRACT

OBJETIVO: determinar las características clínicas e histopatológicas de los tumores en los anexos oculares durante el período 2009 al 2010. MÉTODOS: se realizó un estudio observacional descriptivo longitudinal retrospectivo en un grupo de 166 pacientes con diagnóstico de tumor de anexos oculares, escogidos en consulta de Oculoplastia en Holguín. RESULTADOS: predominó la edad comprendida entre 41 a 50 años, con 37,35 %. El sexo más afectado fue el femenino con 53,61 %. La mayor cantidad de lesiones se encontró en los párpados de 114 pacientes. El tipo histológico mayormente identificado fue el nevus con 25,90 %, seguido por el carcinoma basal con 18,07 %. Se obtuvo una concordancia de 83,13 % entre el diagnóstico clínico y el histológico. CONCLUSIONES: en los pacientes estudiados las lesiones tumorales de los anexos se expresan comúnmente en el sexo femenino con una edad riesgosa a partir de la cuarta década de la vida donde los parpados son los más susceptibles. El nevus y el carcinoma basocelular se comportaron como un problema de salud por su predominante frecuencia, con una buena utilidad del método clínico en su diagnóstico.


OBJECTIVE: to describe the clinical and histopathological signs of ocular adnexa tumors detected in the 2009-2010 period. METHODS: retrospective, longitudinal, descriptive and observational study of 166 patients presenting with some adnexa tumors, who were chosen at the oculoplasty service located in Holguin province. RESULTS: the 41 to 50 years-old group accounted for 37,35 %. The most affected patients were females with 53,61 %. The greatest amount of injures was found in the eyelids of 114 individuals; the mostly identified histological type was nevus which represented 25,90 % of cases, followed by basal carcinoma with 18,07 %. An agreement of 83,13 % between the clinical and the histopathological diagnosis was reached. CONCLUSIONS: the tumor lesions of adnexa found in the studied patients commonly occur in females at risky ages as of the 40 years when the eyelids are more sensitive. Nevus and basocelular carcinoma became health problems due to their predominant frequency, and they were correctly diagnosed by the clinical method.

ABSTRACT

OBJETIVO: caracterizar la correlación clínica y anatomopatológica de las lesiones tumorales palpebrales. MÉTODOS: se realizó un estudio descriptivo y retrospectivo en los pacientes con tumores palpebrales intervenidos quirúrgicamente en el Servicio de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer" en el período de enero a septiembre de 2011. La muestra quedó constituida por 196 pacientes operados de las diferentes tumoraciones palpebrales, quienes cumplieron con los criterios de inclusión. Las variables que se utilizaron fueron: edad, sexo, color de la piel, localización de los tumores palpebrales malignos, calidad de la exéresis quirúrgica y correspondencia entre el diagnóstico clínico y el anatomopatológico. RESULTADOS: el grupo de edad de mayor porcentaje fue el de 65 años y más, con 38 %, así como el sexo femenino con 63 %. Fue mayor el número de pacientes de piel blanca, con 85 %. Coexistió correspondencia clínica y anatomopatológica en el 65 % de los casos con tumoraciones benignas, y en las malignas con el 71 %. En la mayoría de los casos la exéresis de la lesión fue completa. CONCLUSIONES: existe correspondencia entre el diagnóstico clínico y el anatomopatológico de las tumoraciones palpebrales, la cual es superior en los tumores malignos.


OBJECTIVE: to characterize the clinical and pathological correlation of eyelid tumor lesions. METHODS: a retrospective and descriptive study was conducted in patients with eyelid tumors surgically treated at the Ocular Plastic Surgery Department of "Ramón Pando Ferrer" Cuban Institute of Ophthalmology in the period from January to September 2011. The sample consisted of 196 patients operated on from different eyelid tumors , who met the inclusion criteria. The variables used were age, sex, race, location of malignant eyelid tumors, quality of the surgical resection quality and correspondence between the clinical and the pathological diagnoses. RESULTS: the largest age group was the 65 years-old percent and older with 38 %, and females accounted for 63 %. The Caucasian patients represented 85%. There was clinical and pathological correspondence in 65 % of patients with benign tumors and in 71 % of malignancies. Excision of the lesion was complete in most of cases. CONCLUSIONS: there was correspondence between the clinical and the pathological diagnoses of eyelid tumors, being higher in malignant tumors.

ABSTRACT

El carcinoma de glándulas sebáceas es un tumor altamente maligno y potencialmente letal. Puede enmascarar enfermedades inflamatorias como blefaritis, blefaroconjuntivitis, meibomitis, tarsitis y chalazión recurrente. Por eso, ante la presencia de una lesión palpebral de presentación atípica o recidivante se impone una exéresis completa con margen oncológico para su estudio anatomopatológico. Esta afección cursa con un diagnóstico clínico difícil, y en ocasiones inadvertido, por lo que se realizó una revisión bibliográfica para profundizar en sus diferentes formas de presentación, pronóstico y tratamiento.


The sebaceous gland carcinoma is a highly malignant tumor and potentially lethal; it may masquerade inflammatory diseases such as blepharitis, blepharoconjunctivitis, meibomitis, tarsitis and recurrent chalazion. Hence, the presence of palpebral injury of recurrent or atypical presentation imposes a complete resection with oncological margin for pathological examination. This condition presents with difficult clinical diagnosis and sometimes unnoticed, so we conducted a literature review to delve into its various forms of presentation, prognosis and treatment.

ABSTRACT

OBJETIVOS: describir las características clinicopatológicas de los tumores malignos palpebrales. MÉTODOS: estudio descriptivo y retrospectivo de pacientes con diagnóstico histopatológico de carcinoma basocelular, epidermoide y adenocarcinoma de glándulas sebáceas, atendidos en el Servicio de Oculoplastia del Instituto Cubano de Oftalmología, de enero de 2005 a diciembre de 2010. Se incluyó un total de 109 pacientes; se analizó la edad, el sexo, el color de la piel, las formas clínicas, la localización anatómica, el grado de diferenciación celular y la frecuencia anual. RESULTADOS: el grupo de edad entre 60 y 79 años constituyó el 50,5 % de los casos. El sexo masculino constituyó el 47,7 % y el femenino el 52,3 %. El carcinoma basocelular se presentó en el 68,8 % de los pacientes, el carcinoma epidermoide en el 19,3 % y el adenocarcinoma de glándulas sebáceas en el 11,9 %. La piel blanca mostró el 88 % de los casos; el carcinoma basocelular nodular estuvo presente en el 53,3 % y la noduloulcerativa en 33,3 %. En el párpado inferior se presentó el carcinoma basocelular en el 69,3 % y el epidermoide en el 66,7 %. El 44,1 % de los diagnósticos histopatológicos fueron moderadamente diferenciados. Mostró 21 casos de carcinoma basocelular en el 2009; 7 de carcinoma epidermoide en el 2007 e igual número de casos de adenocarcinoma de glándulas sebáceas en el 2010. CONCLUSIONES: más de la mitad de los pacientes que presentan tumores malignos palpebrales son adultos mayores, femeninos y blancos. Se evidencia la totalidad de las formas clínicas de presentación; predomina en el párpado inferior el carcinoma basocelular y el epidermoide. La mayoría de los casos muestra grado de diferenciación celular moderado y la frecuencia anual tiene disímil comportamiento.


OBJECTIVES: to describe the clinicopathological features of malignant eyelid. METHOD: a descriptive, retrospective study of patients with histopathologic diagnosis of basal cell carcinoma, squamous cell and adenocarcinoma of sebaceous glands, assisted in the service of oculoplasty the Cuban Institute of Ophthalmology January 2005 to December 2010. It was included 109 patients; we analyzed age, sex, skin color, clinical symptoms, anatomic location, degree of cell differentiation and annual basis. RESULTS: 60 to 79 years occurred in 50,5 %. It was 47,7 % were male and female 52,3 %. The basal cell carcinoma was present in 68,8 %, squamous cell carcinoma in 19,3 % and adenocarcinoma of sebaceous glands in (11,9 %. White skin was 88 %. Nodular basal cell carcinoma in 53,3 %, the nodule-ulcerative in 33,3 %. In the lower eyelid basal cell carcinoma showed a 69,3 % and 66,7 % epidermoid. The 44,1 % of the histopathological diagnoses were moderately differentiated. Showed 21 cases of basal cell carcinoma in 2009, 7 squamous cell carcinoma in 2007 and an equal number of cases of adenocarcinoma of sebaceous glands in 2010. CONCLUSIONS: over half of the patients were elderly, female and white, showed all the clinical forms predominated in the lower eyelid basal cell carcinoma and squamous cell, most of the cases showed degree of cell differentiation moderate annual rate showed dissimilar behavior in the period under study with more cases in 2009.

ABSTRACT

El cuerno cutáneo es un concepto semiológico. Se define como una lesión tumoral hiperqueratósica de aspecto cónico y tamaño variable, que puede tener como base una lesión de origen benigno, premaligno o maligno. El tratamiento consiste en la resección completa de la lesión, seguido de un estudio histopatológico para precisar el diagnóstico. El carcinoma sebáceo, neoplasia maligna derivada del epitelioanexial de las glándulas sebáceas, se ha descrito como una etiología poco frecuente de cuerno cutáneo. Su diagnóstico es difícil, ya que carece de elementos clínicos característicos y su histología puede ser similar al carcinoma espinocelular o basocelular, pero importante ya que constituye una neoplasia agresiva, con riesgo de recurrencia local y metástasis. A continuación, presentamos el caso de un hombre de 68 años con diagnóstico clínico de cuerno cutáneo palpebral y diagnóstico histopatológico de carcinoma sebáceo.


The cutaneous horn is a semiological concept. It is defined as an hyperkeratotic tumor of conical appearance and variable size, which may be based on benign, premalignant or malignant lesion. The treatment is the complete resection followed by an histopathology study to clarify the diagnosis. Sebaceous carcinoma, malignant neoplasm derived from adnexal epithelium of sebaceous glands, has been described as a rare etiology of cutaneous horn. Diagnosis is difficult because it doesn’t have characteristic clinical features and histology may be similar to squamous or basal cell carcinoma, but it is important because is an aggressive neoplasm with risk of local recurrence and metastasis. We present a case of a 68-year-old man, with a clinical diagnosis of eyelid cutaneous horn and histopathological diagnosis of sebaceous carcinoma.

ABSTRACT

Sebaceous carcinoma of the eyelid is a very rare slow-growing tumor and is considered an aggressive eyelid neoplasm. It can reach mortality rate of about 6 percent. Diagnosis is often delayed because of its ability to masquerade as other periocular lesions, both clinically and histologically. We present three cases of sebaceous carcinoma, with different surgical outcomes, showing the importance of early diagnosis.


Carcinoma sebáceo é um tumor raro de crescimento lento considerado uma das mais agressivas neoplasias palpebrais. Pode alcançar taxa de mortalidade de aproximadamente 6 por cento. O diagnóstico tardio é comum devido a sua habilidade de se confundir com outras lesões perioculares tanto clinicamente quanto histologicamente. Relatamos três casos de carcinoma sebáceo da pálpebra com diferentes resultados cirúrgicos, enfatizando a importância do diagnóstico precoce.

ABSTRACT

O retinoblastoma pode acometer um ou os dois olhos, ocorrendo de forma hereditária ou esporádica. O portador dessa doença pode desenvolver, a longo prazo, outros tipos de tumores não oculares. Relatamos dois pacientes: o filho, portador de cavidade anoftálmica bilateral após remoção dos olhos devido ao retinoblastoma, que apresentou segundo tumor palpebral unilateral, afetando a pálpebra superior; e o pai, portador de cavidade anoftálmica unilateral também por remoção de retinoblastoma e que apresentou tumoração de crescimento rápido na pálpebra superior ipsilateral ao olho enucleado. O exame imuno-histoquímico de ambas as lesões excisadas revelou que se tratava de carcinoma de glândulas sebáceas. Os pacientes evoluíram bem após a remoção do tumor palpebral e, até o momento, não apresentam sinais de recidiva ou metástase.


Retinoblastoma is a unilateral or bilateral intraocular tumor, occurring as a hereditary or sporadic tumor. Another not ocular lesion is a possibility for retinoblastoma carriers. The authors report the cases of two patients: a son with bilateral anophthalmic socket due to retinoblastoma, who presented unilateral upper eyelid tumor; and his father with unilateral anophthalmic socket secondary to enucleation due to retinoblastoma and a tumor growing fast in the upper eyelid at the same side of the anophthalmic cavity. Lesions were diagnosed as sebaceous carcinoma confirmed by immunohistochemical exam. Both patients had the eyelid tumors removed, with good resolution. None of them have presented either signs of recurrence or metastasis.
